The Information form asks for basic data about your and your family, including your medical history.  It also contains a disclosure form that outlines my policies on issues such as confidentiality, scheduling, fees, and insurance practices.

The HIPPA Consent form is not a release of information.  You would need to fill out a separate form to allow me to disclose your information to a third party.

The HIPPA Privacy Notice is a long list of the protections and limits to protection of your private information.
