Fees will be stated in the Consent for Treatment Form, and payment is due at the time of service by check or through Zelle.
I am not on any insurance panels, but I will give you a superbill that you can submit to your insurance company for reimbursement to you.
I have opted out of Medicare, so they cannot be billed, as well as any secondary insurance carriers.
