Fees are stated in the Consent for Treatment Form, and payment is due at the time of service by check or through Zelle at [email protected].
Confidentiality of what you disclose in therapy is extremely important to me. One of the ways I protect your sensitive information is to not be contracted with any insurance companies. Of course, if you want to submit a claim for reimbursement consideration, I will provide you a superbill that you can submit to your insurance company on your behalf.
I have opted out of Medicare, so they cannot be billed, as well as any secondary insurance carriers.
